2. What this notice covers
This notice covers people who visit this website and people who contact us, usually dentists and practice staff.
It does not cover patient information that a dental practice sends us with a case, such as a prescription, impressions or scans. The practice is responsible to its patients for that information, and we handle it under our arrangements with the practice. Patients with questions about their information should contact their dental practice first.
3. The information we collect
Information you give us
- Contact form: your name, practice name, email address and your message.
- Price list and guide requests: your name, practice name, email address, phone number if you give it, the items you ask for and any notes.
- Emails, calls and digital files: anything you include when you email us, call us or send files to sapphirelabstl@gmail.com.
Please don't include patient details in the website forms. Send case information through your usual route to the lab.
Information collected automatically
- Server logs: our hosting provider records technical details of each request, such as IP address, browser type and the page requested, to run and secure the site.
- Visitor statistics: we use Vercel Web Analytics to count page views. It doesn't use cookies and doesn't identify individual visitors.
4. How we use it and our lawful basis
- To answer your enquiry or send what you asked for, such as the price list, lab request tickets or lab bags. Our lawful basis is our legitimate interest in responding to businesses that contact us, and, where you are asking about working with us, taking steps at your request before entering into a contract.
- To keep a record of our dealings with you. Legitimate interests.
- To run, secure and improve the website, including blocking spam sent through the forms. Legitimate interests.
- To meet legal obligations, such as keeping business and tax records. Legal obligation.
We don't send marketing emails unless you ask us to, we don't sell your information, and we don't make decisions about you by automated means.
6. International transfers
Some of these providers store or process information outside the UK, including in the United States. Where they do, the transfer is protected by UK adequacy regulations or by contractual safeguards approved under UK law, such as the International Data Transfer Agreement or Addendum. Contact us for more detail on the safeguards for a particular provider.
7. How long we keep it
- Enquiries that don't lead to work: for 2 years after our last contact.
- Records of work for practices: as long as we need them for that work and to meet our legal, tax and regulatory obligations.
- Server logs: for the limited period our hosting provider keeps them.
8. Your rights
You have the right to:
- ask for a copy of the information we hold about you
- ask us to correct information that is wrong or incomplete
- ask us to delete your information
- ask us to restrict how we use it
- object to us using it on the basis of legitimate interests
- ask for information you gave us to be transferred to you or another organisation
To use any of these rights, email info@sapphiredentallab.com. We'll reply within one month. There is normally no charge.
9. Keeping it secure
The website is served over an encrypted connection (HTTPS), and we limit access to personal information to the people who need it to do their work. No method of sending information over the internet is completely secure, so please don't send patient details through the website forms.
10. Complaints
If you're unhappy with how we've handled your information, please tell us first so we can put it right. You can also complain to the Information Commissioner's Office (ICO) at ico.org.uk/make-a-complaint or on 0303 123 1113.
